Here is my options file:

/dev/modem
38400
crtscts
defaultroute
name <my user name>

I tried adding some stuff from a newer options file I found:

asyncmap 0
auth
lock
modem

Still, PAP does not work. The modem sounds like it has connected, then it
disconnects, assumedly because PAP is not working.
