Uhm.

DirectX does this stuff by essentially taking over control of the video
device.

OpenGL does this via the FS(Full screen) extension.

In the case of opengl all it does it make a window without borders and
slams it into the upper left corner(at least, on my tnt it does) and asks
for the resolution to be dropped to X and be positioned over it. I can
tell because I sometimes have applications that are "always on top"(ie,
winamp) that sometimes poke their little edges into the side.
