Have you tried isapnptools? If not, then I'd suggest finding out which irq, etc.
the modem uses under win, do a pnpdump, and set the parameters in your
/etc/isapnp.conf. Most distriubutions check for the existence of the file at boot
and run isapnptools at startup if it is there.
